Lupron Depot® Co-pay Savings Card
Patients with commercial insurance coverage for Lupron Depot® (leuprolide acetate for depot suspension) 3.75mg will pay as little as $10 for their co-pay, up to a maximum savings of $300 per month. Simply download a savings card and present it to MDR along with your prescription for Lupron Depot®.

- Lupron Depot® (leuprolide acetate for depot suspension) 3.75mg
Eligibility Requirements
- Valid for patients with commercial insurance coverage for Lupron Depot®
- Not valid for patients receiving prescription reimbursement under any federal, state, or government funded insurance plan.
- Indication for endometriosis or anemia prior to uterine fibroid surgery
Enrollment Instructions
- Go to www.luprondepotsavingscard.com.
- Answer the questions by selecting your indication and confirming that you have commercial insurance coverage.
- Download your co-pay savings card.
- Present the co-pay savings card to MDR when you’re ready to pay for your prescription.
